Each of zombie and skeleton wandering the map has name of a member of Reaper of Souls dev team, with their role instead of creature type. The second floor of the rift is another Briarthorn Cemetery map, but this time it has no Elites at all. The Rift Guardian is a Unique Punisher called Josh Mosqueira (the game director of Diablo III at the time of Reaper of Souls release). You can typically spawn the Rift Guardian after clearing about 70% of the first floor. The first floor is medium-sized Briarthorn Cemetery map with its native mobset and insane Elite density. These rifts have fixed maps and unique Rift Guardians. Hope that helps, i'm only paragon 450 right now, but I did it fast and furious. Each time you open a Nephalem Rift there's a small chance that you get one of the two special rifts. You want maximum return for your time when you are farming to progress. Anything greater than 5 minutes on a Neph Rift can be a waste of time. If you aren't able to complete a Neph Rift in under 5 minutes: Then go down a difficulty level. You'll find that sometimes elemental damage increase can be better than raw Damage upgrades. Look at the element of the attack you like and try to boost that. If you find you are not doing enough damage: Get gear that directly boosts the type of skills you are using. Also any talents that prevent insta-death. This will increase your overall chunk and help. If you find you are getting 1 shotted: Get better armor rating / vitality. I was a mixed bag of armor and talents lol. Those little tweaks help you get from 10-13. When you get to T10, then you can start min/maxing really. I wouldn't put too much on any specific gear, but more so what gear do you need to help your survivability in this difficulty? I found that moving through the Torment levels, that I was constantly changing my build/gear for what ever difficulty I was able to farm.
